Foundational Support Package (about 3 months)

Because nutritional therapy is not a quick bandaid fix and it takes time to implement changes, I believe 3 months is a reasonable amount of time to get started and see the positive impacts that nutritional therapy can have on you! There will be an option to extend our time working together if needed.

What this foundational support package includes :

* A 90 minute initial consultation/interview going over the Initial Interview Questionaire, NAQ assessment, & 3 day food & mood journal

* * 3 - 45 minute recommendation visits going over your personalized diet/lifestyle recommendations (one set of recommendations each month)

* biweekly follow up visits (6 @ 30 mins)

* Unlimited email support

* Educational flyers

* Balanced meal ideas

What's expected of you ?

Filling out the initial interview questionaire, NAQ assessment (320 questions), and food & mood journal in a timely manner in the beginning, and again toward the end of our 12 weeks. Every 4 weeks, I will have you fill out a follow up form and a 3-4 day food & mood journal.
